SRR Boarding Check In Form

Please take a minute to fill in the following info prior to checking in for boarding

It is important to double check you've selected correct dates/times, as well as refer back to confirmation if you forget dates/times. After we receive & review check in form, we will send a confimation copy back to you via email.

We try to be flexible for adjustments to days/times if we are able. We will not accommodate check ins/outs outside 7am-5pm.

  • If you choose NOT to add on services, your dog will go on 20 min potty breaks 5x throughout the day. No dog to dog play or yard time.  No text updates.

  • If you chose "Playtime; dog to dog play" we offer multiple sessions throughout the day, when good play partners are avaliable. We do not offer a "duration" on sessions. Each dogs needs vary, each play session ends when pups are ready to rest and needs have been met. (if we do not have good play partners, we will adjust to exercise only on those days) Intermittent text updates

  • If you select "Exercise only: no dog to dog play”. You may select this for various reasons: dog selective, injury or senior dog, etc. We are still happy to provide individual exercise for your dog in whichever form they prefer most, fetch, walk or cuddles! Each dogs needs vary, each exercise session ends when pups are ready to rest and needs have been met. Intermittent text updates.

Reminders for Boarding-​

  • Check in & out: Arrive at scheduled time! We allow clients to choose times to be accommodating travel arrangements. Please arrive on time, we often are stopping cleaning/playdates/private training appointments to complete check in's and out's. If repeatedly late, $1/min fees will be charged to invoice.

  • Clients must complete Check In Forms at least 48 prior to check in. Ideally complete check in forms as soon as you've confirmed desired reservation dates and times.

  • Cancellation Fees in affect for Holidays & within 48 Hours to scheduled Check in Day: will result in 30% cancellation fee of total stay scheduled.

  • Females are Not to Board During Heat Cycles. If they start their heat cycle you will be asked to pick up.

  • Dogs are NOT to board if ill (coughing, vomiting, diarrhea, etc)

  • No Human Aggressive Dogs

We really want to ensure all dogs boarding are comfortable, pending developments during stays, we may require all natural calming meds or prescribed anti-anxiety meds from your vet.

WAIVER FORM

 

 

I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are attending SRR Woof Academy & Stables LLC.

 

I also understand and agree that in admitting my dog(s), SRR Woof Academy & Stables LLC. has relied upon my representation that my dog(s) is/are in good health and have not harmed or shown aggressive or threatening behavior towards any person or any other dog.

 

I further understand that due to the way that dogs interact with one another, minor cuts and scratches can occur even though the dogs are carefully supervised at all times.While my dog(s) is/are in the care and custody of SRR Woof Academy & Stables LLC. if I am unreachable in the event of an emergency, I hereby authorize SRR Woof Academy & Stables LLC., its agents, and/or representatives to seek immediate veterinary care for my dog. I understand that all costs in connection with, veterinary, medical or other treatment shall be my responsibility.

 

I hereby release and agree to save and hold harmless, SRR Woof Academy & Stables LLC., it’s directors, officers, shareholders, employees, assistants, members and agents from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). I specifically, without limitation, agree to fully indemnify SRR Woof Academy & Stables LLC. for any and all such liability, claims, suits, actions, losses, injury or damage.

 

I certify that I have read and understand the rules and regulations set forth herein and that I have read and understand this agreement. I agree to abide by the rules and regulations and accept all the terms, conditions and statements of this agreement and confirm the truthfulness of the contents of the Application form completed by me. Although we carefully screen all applicants, occasionally we discover that this is not an appropriate environment for every dog. 

 

SRR Woof Academy & Stables LLC reserves the right to permanently remove a dog from services Cancellation Fees in affect for Holidays & within 48 Hours to scheduled Check in Day: will result in 30% cancellation fee of total stay scheduled.
